Florida Adoption Home Study

The following documents are needed to complete your Florida adoption home study. It is a good idea keep a copy of all documents for your records. We can schedule your home visit while you are gathering paperwork. You should give your paperwork to your social worker during your visit, but if you do not have everything finished at that time, no problem! You can always mail, fax, or scan/e-mail it to us!

  1. Child Abuse Clearance

    Please fill out this form and give it to your social worker at your home visit. Each household member age 18 years old and older will need to fill one out.

    *If you have lived anywhere other than Florida in the last five years, please let your social worker know ASAP. Additional clearances may be required.

  2. Local Background Check

    Either call or go to your local county sheriff’s office and ask for a local background check. This is separate from fingerprints. Some counties may even offer this service online. This is needed for anyone 12 or older in the home.

  3. Livescan Fingerprint Clearances

    Each household member age 18 years old and older will need to complete fingerprints.

  4. Adult Medical Form

    Please use the above medical form. This is needed for anyone 18 or older in the home.

  5. Child’s Medical Form

    Please use the above medical form. This is needed for any children living in the home.

  6. A photocopy of your most recent tax return

    We need the first two pages of your 1040.

  7. An employment letter

    Please provide an employment verification letter written on company letterhead. This letter should simply state your name, position, full-time or part-time, length of employment or date of hire and current annual salary. The letter should be signed by your supervisor.

  8. Financial Statement

  9. Five Reference Forms

    You will need at least one relative (or one for each prospective adoptive parent if you are adopting as a couple) and at least three must be from non-relatives (four if you only have one relative). If you are married, you should choose references of people that know you as a couple.

  10. Photocopies of birth, marriage and divorce decrees

  12. Affidavit Of Good Moral Character

    This is needed for anyone 18 years or older in the home.

  13. Confidential Home Study Inventory/Application

    Please fill this out in full before your home visit.

  14. Arrest Disposition

    If you have been arrested for any reason, at any age, even if the charges have been dismissed, sealed or expunged, the arrest will need to be addressed in the home study.

    If you have an arrest record, call the Clerk of the Circuit Court in the county where the arrest took place and ask for a certified copy of your arrest disposition or a certified letter stating they no longer have the records on file.

  15. Acknowledgement of Firearm Safety

  16. Department of Juvenile Justice/FDLE

    This is required for anyone age 12-17 in the home. Please go to this website https://www.fdle.state.fl.us/criminal-history-records/florida-checks and click on “instant check”. The results will be e-mailed to you. You can print or forward to your social worker.
